A bus accident in Kokoszki

May 2, 1994 was another dark day in the history of Poland. The PKS Autosan bus drove into a roadside tree 500 meters before the end stop. More than 30 passengers were killed, over 40 were injured. Why did this tragedy happen? Could it have been prevented?
